The Big Idea
After a hospitalization, people need time at home with an aftercare plan to recover and heal. However, people experiencing homelessness are more likely to end up back in the hospital because they lack access to a safe place to stay and supportive medical care. Hennepin County Medical Center will organize a task force of community experts and partners to explore how to support people experiencing homelessness upon hospital discharge. They hope to fully identify gaps in the continuum of care and design a solution to this community need.
